Home Medicines Review
Home
Medicines Review (HMR) is a service provided to patients
living at home within the community. On GP referral, an
accredited pharmacist conducts a medication review in a
patient’s home and reports the findings and recommendations
to the GP. The patient is recalled for a consultation with
the GP to decide on a medication management plan.
To
maximise an individual patient’s benefit from their
medication regimen, accomplished through a team approach
involving the GP and the patient’s preferred community
pharmacy, with the patient as the central focus. Any person
living at home in the community or in a retirement village
is eligible for a Home Medicines Review (HMR) once in a
12 month period (unless there is a significant change in
condition or medication regimen). HMR is not available to
inpatients of a hospital or care recipients in residential
care facilities, including hostels.
