Home Medicines Review

Home Medicines Review (HMR) is a service provided to patients living at home within the community. On GP referral, an accredited pharmacist conducts a medication review in a patient’s home and reports the findings and recommendations to the GP. The patient is recalled for a consultation with the GP to decide on a medication management plan.

To maximise an individual patient’s benefit from their medication regimen, accomplished through a team approach involving the GP and the patient’s preferred community pharmacy, with the patient as the central focus. Any person living at home in the community or in a retirement village is eligible for a Home Medicines Review (HMR) once in a 12 month period (unless there is a significant change in condition or medication regimen). HMR is not available to inpatients of a hospital or care recipients in residential care facilities, including hostels.

  • Those taking many medications or confused by generics
  • Those who are experiencing adverse effects
  • Those who may require education regarding the use of equipment such as puffers or nebulisers. This includes children.


Outcomes include:

  • Achieves safe, effective and appropriate use of medications by detecting and addressing medication-related problems that interfere with desired patient outcomes.
  • Improves patients’ quality of life and health outcomes using a best practice approach involving collaboration between GP, pharmacist, other relevant health professionals and the patient (and where appropriate, their carer).
  • Improves patients’ and health professionals’ knowledge and understanding about medications.
  • Facilitates cooperative working relationships between members of the health care team in the interests of patient health and well-being.
