Health Assessments
Patients
from the age of 75 years; or Indigenous patients from the
age of 55 years who are still living at home are eligible
for a Health Assessment on a annually bases. This service
can only be provided with the patients consent and is not
provided to patients in nursing homes or hostels.

General
requirements
A Health Assessment should generally only be undertaken
by the medical practitioner, or a practitioner working in
the medical practice who has provided the majority of services
to the patient over the past 12 months. Information collection
component of the assessment can be conducted by a nurse
or another assistant in accordance with accepted medical
practice, acting under the supervision of the GP.
The
other components of the assessment must include a personal
attendance by the GP. In a situation where a patient’s
usual medical practitioner does not undertake the health
assessment, a copy of the Health Assessment report should
be forwarded to the medical practitioner subject to the
patient’s agreement.
The
GP should offer the patient a copy of the Health Assessment
or the recommendations.
