Based Care helps to improve patient satisfaction
and for those doctors who may be unfamiliar or hesitant
to embark upon health assessments, we can provide proven
systems which will ease the transition. Home Based
Care also provides services for improved awareness
and utilisation of other EPC items such as Team Care Arrangement,
GP Management Plan and HMRs.
